If an employer provides car service for its staff, does accountant receive documentation of what time employee ordered cab?

... or is documentation only when the cab reaches destination?

It should be documented on the credit card that was paid with.

It depends on how your company has the paperwork set up.  You say car service and later say cab, but I'm going to assume you mean a company that your company has worked out an arrangement wherein they will send a car when called for and then bill the company. I would say they would most probably get the info of date, time, mileage. Or maybe even specific pick up/drop off locations.

Highly unlikely, date however, is relevant.

All that the bothers the accountant is how much the trip has cost.  The details of it are not his concern.
