How should a female pastor be addressed on envelope, such as when mailing her a “thank you” card?
Pastor Jane Smith?
Ms. Jane Smith

And in the thank you card, should it say “Dear Pastor Jane”?

Well, what title does she use? If "Pastor" is the one, then she's "Pastor Jane Smith". In my denomination, she'd be "The Reverend Jane Smith," just like a man in her position.
Her sex is irrelevant.

Protocols for addressing clergy vary by denomination, but NOT by gender. If you use "Pastor" or a similar title for a man, you use it for a woman also. (As for the form of address in the salutation, it should be at least as formal as you would address her in person. My own church's rector does not like being called by her first name.)

All of the above would be appropriate. Pastor's aren't typically hung up on titles. Because it is a thank you card is informal, it is best to leave out the Ms.

On the envelope: Rev. Pastor J Smith
On the card: What you would normally say if you were speaking to her. Dear Pastor Jane or Dear Jane would be fine.
