Depends. If we've met and have been talking and have established some obvious mutual interest it seems natural to exchange numbers. But if a guy just walks up out of the blue to ask this it's usually creepy.

No, not if he was polite, but then your idea of polite and my idea of polite might not be the same.

Yes. It was a friend who wanted to be more and when I said no he kept asking me. Eventually he told me that we should "get a coffee and talk about it". It was still a no. He's an alright guy, can't take no for an answer though.
