When you get the COVID19 vaccine, do you get a document stating you did?

In the US you only get a paper card with the name, date, type of vaccine, lot number. But it does not have any sort of verification by any official or any protection against forgery or modification.

Here in the us, it’s a card from the CDC with lot number date etc on it.
