There are no "native" Americans. So-called "indians" came from Asia. Humans did not evolve in the new world. NO humans are native to the Americas, NONE.

If you're born in the USA your nationality is American, but American ethnicity denotes the indigenous peoples of the Americas.

You can be a US citizen but the only "American" ethnicity would probably be those umbrellaed under the designation Native American. Although even still they're more likely to cleave to individual nations/tribes as not all Native nations consider themselves related as one big group.

Ethnicity has a racial connotation. What they want to read is black, brown, red, yellow, or white, or the political correct form of it, as well as Pacific Islander, Eskimo, etc. But you are totally in your rights to write American.

Ethnicity is a collective noun and means cultures, customs, affiliations and associations YOU have from your country of birth and/or where you were brought up, your home/family... so the food YOU eat, the holidays YOU celebrate, the sports team YOU follow. the language YOU speak........ so of course
But in the singular form, an ethnicity is a particular 'ethnic' group...so if your parents were born/brought up in another country...then they will expose you to THEIR own ethnicity, so you may also have 'some' other ethnicity such as you speak another langauge, eat different food they were brought up eating etc etc..
