    This is an embedded question. When you have a question with two clauses like this one, you only invert the subject and the verb in the first one.

    "What MADE YOU first realize"-- the verb comes before the subject, which makes it a question.

    "you were in love"- the subject is before the verb in normal sequence.

    Similarly:

    Can you tell me what the time is?

    Do you know where he went?

    The first one is. Add the word "that" after realize.
