Words and gestures have meaning. Somebody wanting to say no but not saying it doesn't convey any meaning unless you're a mind reader. If you're asking "why would somebody nod their head yes when they want to say no?" it means that they are reluctant to say no for any number of possible reasons, such as fear or not wanting to offend.
