How do you differentiate between someone non-binary and someone with split personality if they're called "they"?

    Context is how you determine the reason someone is being referred to as "they". 

    Of course, those aren't the only two possibilities. A common reason for referring to someone with 'they' is that their gender is unknown.

    You simply identify them as the sex that you can see they are, which has been what people have been doing for our entire human history.  Not by any "gender" identity, not by any sexual identity, not by racial identity, not by their mental personal identity, all of which we can never do anyway.  Those require getting into someone's mind.  Sex on the other hand is a physical, biological reality that is concrete.
