    The main reason is that the person did not pay attention to the directions and instructions.

    The second reason is that the manager/supervisor gave the instructions incorrectly or not at all.

    They don't know any better.

    Not paying attention to what their doing.

    Not double checking their work.

    Being too complacent.

    Rushing too much to get things done, faster isn't always better.
