individual applies to percentages, so 12 also does.  the intervening adjectives (or noun as adjective), individual and completion, also apply to percentages. 12 does not apply to individual any more than individual applies to completion.

12 percentages, individual percentages, completion percentages, 12 individual completion percentages.  much like 12 running dogs.  not 12 running, but 12 dogs, which are also running.
