What is the common interface to switch the fog lights on/off in American cars?

GMC 1991. Are cars equipped with fog lights by default or not?

There is no 'common interface' between cars - and there is no 'default' for fog lights.  Some come equipped with them, and some don't.

The last car I had with fog lights was an '08 Focus ZX5 - and you turned them on by pulling on the headlight switch.

If your GMC wasn't equipped with fog lights, and you decide to install a set (or have them installed) there may already be wiring/fuse for them, or you may have to install everything.  There might be a blank cover somewhere on your dash to install  the switch, or you might have to swap out one of the controls to include the fog light switch (or mount a switch someplace else).

If you're talking about the additional red tail light(s), bright as a brake light, that is mandatory in Europe for visibility in fog, they are not required in the US and so they're not found on American-spec cars. Forward fog lights are also not standard equipment, they are ordered as an add-on or included in premium trim levels.

However, GM did start using daytime running lights (always-on headlights) in 1995, standard on all models by 1997.

American cars generally do not come from the factory with fog lights. Many people added them in the 70's. For off road trucks there are aftermarket roof mounted light bars.
