In the many countries where smoking is still acceptable, not just Japan, those countries didn't sign up to the world health organisation's drive to reduce smoking. They had other priorities but in the countries that have let WHO in, the anti smoking message is doing really well and a lot of people are stopping. I live in the UK and in 1993 I was in a children's home, staff were allowed to smoke in the same room as the kids back then, which isn't that long ago and nowadays that would be shocking.
