If my migraines are managed with medication, but Unmanageable without it, does that still count as “unmanageable” pain, for diagnostics?

    No.  You are managing the pain using those pill.  It is manageable.  Real unmanageable pain could not be stopped by any sort of therapy including pills.  That's why it's call "unmanageable."
