The coaching staff for not replacing the kicker, the holder and/or the snapper.

It can happen for any reason.Bad snap, holder boobles it,lineman not doing his job, kicker not as fast as he should etc..etc.With all that said I believe most of the time is on the holder

A blocked field goal is generally the result of one of a couple things.

    1) A bad snap disrupts the timing of the kick causing it to come out late.  FG protection is timed to only protect for a second.  If something disrupts the timing of the kick, not only does it make the kicker miss a lot, but it can lead to blocks.

    2) The kicker is getting the ball out low.  if the block is the result of someone jumping up in the middle, it's probably a low kick.

    3) Edge blockers don't need to do a lot, but they need to at least get a solid chip on edge rushers.  If you see someone come in from the edge, it's the edge blockers.

But ultimately, all of these are easily correctable.  If you're getting FG's blocked a lot, it's on your special teams coach.  He's failing to get his guys to do the job.

I'd say partially depends on how it's being blocked. If it's being blocked over the line then that's the kicker not getting it up enough from teh start. If it's being blocked at the foot then that's the line. Either bad snaps slowing down teh holder or more likely a lineman that can't hold their own and keeps getting beat.
