Who decides what is good etiquette?

I mean, what is the standard.  Does it vary among cultures or countries?  Is any of it actually ethical?

    Etiquette varies across cultures and throughout history, etiquette has been defined by different and diverse social standards/settings.  Generally, good etiquette is considered interconnected and collectively understood rules implying acceptable/positive impressions as we each engage in social and business scenarios.  These rules have evolved over time, and continue to reflect current social norms and ethical codes.
