why do I hear Trumps words from his mouth when he answers questions and a few minutes later on media they are taken out of context?

Because no one understands half of Trumps babble, and the other half is too embarrassing for any intelligent person to repeat.
So people have to try and paraphrase him, without sounding like utter morons.

Do you have an example? For me, the problem is that he keeps changing the words from his mouth. Like, he said he did not pay off Stormy Daniels, and then a few weeks later he said he did. And two weeks ago, he tweeted in all caps "SOCIAL DISTANCING!", and now he wants us to go back to work. So it isn't a context problem for me. It is a problem with the words that actually come out of his mouth.
