What should you do when someone refuses to transfer title so to make you keep paying for their insurance?

Car's still in my name.

    If you got a signed bill of sale with the date on it just keep it and quit paying the insurance. If you didn't get a bill of sale signed by the buyer get an attorney.

    Go to the DMV and submit a notice of transfer and release of liability.  Cancel the insurance after doing so.

    Cancel the insurance. It's not your car, and not your problem once you report the sale to the state yourself... which you should have done the minute you signed the title. If the car gets impounded or earns a fistful of parking tickets and they just shrug their shoulders and leave it behind, guess who's left holding the bag.

    I would slit their throat and let them bleed-out in the street. 

    You however might want to go to the DMV and report the car sold (date time and person and turn in the tags and registration), then drop the insurance.
