    It means you are aware of people's needs and address them, well before they express those needs

    He/she waits until they come across someone who has issues and then helps or takes advantage.

    It's not a normal thing to do unless your profession has something to do with it. People who anticipate the needs of others are: waiters, maids, therapists as a few examples. If you are just a regular person who knows an issue will arise so you can jump to it is weird. It's like they look for people who have low esteem and now will know they will admit to their issues so they can help but mostly it's to take advantage as they are not up to no good.
