When you are writing a manuscript and include and italicized word, are punctuation marks attached to the word italicized as well?

So say you write the word Really? with a question mark as part of dialogue. You want Really to be italicized. Do you include the question mark, or is it in normal font?

Update:

title should read 'an italicized word' not 'and italicized word'

    If the character had said, "Did that *really* happen?" and only "really" was stressed, the question mark would not be italicized.

    But she said only one word, and she stressed it. There is no space between the word and the mark, so for appearance, the question mark is italicized.

    If would be in the flow of the words how you would write using this as an ending word....."Really".....but probable with a question mark for it is a one word question when used at the end of a sentence.....
