I wouldn't say "Hi Lara" in writing to my boss.  I'd just say "Lara"  etc.

The rest of your message is basically okay.  Just a couple of changes

    Lara,

Thank you for approving my half-day off for the 30th.  However I just realized that my appointment is for Friday, January 31st at 1:00pm.  Could I please take my half-day on Friday instead of Thursday?  Thank you.
