How does one get off the boat early if it has not docked locally yet or scheduled to dock?

Pilot boat, if laws and your captain permits.  But usually when docking in a country not your own (or even your own) you must present documents for identification to customs and declare any goods.

In some medical emergencies, maybe a helicopter extraction (even without a landing pad) could be an option.

Source(s): Was on a geophysical crew on a ship all over the Caribbean (Panama, Jamaica, Trinidad). Had to see local official before leaving ship for any reason.

Swim for it?  Personally I would wait until it docks.
