If an employee has social anxiety, i.e. a “hi” and “bye” kind of person who’s too shy to have a conversation, is it not rude to ignore him?

Let’s say he’s unable to make eye contact, and doesn’t stop to talk to any of his co-workers, but instead smiles and says “good morning” or “how are you doing?” (not actually asking, but just his way of saying hello) and keeps on walking.  One of his female co-workers always ignores him and never smiles at him, as if he’s invisible.  Is she doing that to accommodate his social anxiety, as he’s too scared to speak to people, so she doesn’t allow him to feel obligated to say “good morning” to her, therefore ignoring him in an effort to let him know he doesn’t even have to acknowledge her.  Or does she think he’s weird and creepy because of his eyes looking left and right, and doesn’t feel comfortable saying literally one word to him?  

Or perhaps she thinks he’s rude since he never speaks to anyone except to follow friendly protocol.

    He is at least making the attempt to be polite and friendly. And in the way you have described it, he is neither creepy or rude about it. So for her to completely ignore him like that, comes off as extremely rude on her part.

    She is rude.  Everyone seems to know he suffers from anxiety.  No reason she can't say hello and good-bye; it's only good manners.  He isn't being rude given his condition.
