direct copy or translation. Each word is translated rather than the entire phrase. The problem is that words have several meanings and even context matters to the meaning, so just lining up equivalent words in the same order will not make a sentence with the same meaning. We Americans are particularly familiar with instructions from Japanese manufacturers that are word-for-word translations of those instructions from Japanese originals, and thus make absolutely no sense in English. the Japanese does not translate word for word.
It is often quite amusing when they employ the wrong equivalent words. Usually, though, it is just incomprehensible.
When talking about making copies of something (one use of the phrase word-for-word), word for word has the same meaning as letter for letter or letter by letter. It means that what is in the original is duplicated exactly.
