If a nurse opts out of union dues are they still represented by the union? Does the nurse still have voting rights?

No.  And no.  They have opted out of the union.

Not many people know this, but in the twenty-four states with "right-to-work" laws, you can opt out of union dues.

If they are not a fee paying member then they have no voting rights, if the union negotiates better pay/conditions it is likely they would also be included in getting those

No dues paid, then not a member of the union and no protection or voting rights.

You have to be a member to vote and to be represented.

Even if you opt out, some contracts require you to pay an administrative fee, as you still benefit from the union. Examples are insurance, wages, and other contract negotiations. The administrative fee could be just as much as union dues.

If there is a collective agreement they would still be represented by the union as a freeloader but would not have voting rights.

    Of course not!

You want the benefits of union membership - you pay your dues!

My guess is that you would negotiate your salary with employer on your own instead of as part of a group.

I know thousands of people who have successfully negotiated their own salaries without the help of any union.
