Could my sister get into an Ivy League school?

She is a sophomore with a weighted GPA of 98 or 99, and she never studies for anything. She has 99s or 100s in Honors Algebra II and Honors Chemistry (and never studies). She got a 1440/1520 on the PSAT this year (which is the 99th percentile). She is definitely in the top 5/100 kids in her grade, and probably the top 2. She does student council, Girl Scouts, theater, choir, and mock trial.

Ivy League schools get thousands of applications like her’s and better.   They want a student to be outstanding, done something unique, have had leadership positions in all they do. Harvard’s latest acceptance rate is 4.5% with 1,950 of 43,330 candidates securing places.

Not studying for anything is not good. Once at a university of the Ivy League level she will need to have great time management skills and study skills.

She should be going on college visits soon. And not just focus on Ivy League schools.

yes. she has a fair chance. it will be much improved if she applies to multiple ivy league schools since most of them accept fewer than 1/10th of qualified applicants

A big "maybe." See, every high school has a handful of students like her, but the Ivy League schools cannot accept them all. There aren't that many slots.
