DMT2 care plan help?

I am not sure if this care plan is correct. Please give me feedback on how/what needs to be changed.

Nursing diagnosis 1: risk for unstable blood glucose.

Related to: inadequate blood glucose monitoring.

Manifested by: did not monitor/control BG prior to hospitalization.

Nursing diagnosis 2: risk for metabolic imbalance

Related to: disease process (DMT2)

Manifested by: no adherence to previous diabetes management plans

Nursing diagnosis 3: ineffective health maintenance

Related to: poor diet, unhealthy lifestyle, unmanaged diabetes

Manifested by: eats foods high in sugar, not active, does not monitor BG

Nursing diagnosis 4: deficient knowledge

Related to: Disease process (DMT2)

Manifested by: unaware of hazards of hyperglycemia

    There were a couple of words/terms that were too subjective:Poor diet,Unhealthy life style , eats foods high in sugar[ this is not a cause ] We manage carbohydrate and glycemic load.

    DMT? You mean like ayahuasca? It’s hallucinogenic.
