How does a therapist connect with a reluctant/distrusting patient?

Well, you don't.
Remember that you CANNOT motivate a patient who is noncompliant with treatment to suddenly start taking his or her illness seriously, roll up their sleeves and actually start to work on their problems.....ESPECIALLY IF THEY HAVE SOMEONE IN THEIR LIVES WHO WILL ENABLE THEM AND FINANCIALLY SUPPORT THEM as they become dysfunctional.
You will have to wait until their enabler decides to stop allowing them to be ill.

Some patients have said that they get a lot out of therapy sessions because the therapist sympathizes with them and understands why the patient is disturbed.
