Does keeping a phone powered off most of the time make the battery last an extra year or two before it will need replacement?
How long till a battery needs to be replaced if the phone was always powered off?

Yes. Cell phone batteries only last between between 300 to 500 charging cycles. If you have an Android phone, it's not a big deal because a replacement battery is about 40 dollars and it's easy to install.

the battery always has some drain to keep the internal clock &c. running. Probably it will go flat in a couple of months. If you really want to keep a phone as a ready to go unit you need to give it say 20 minute on charge every 2 weeks or so. If you let it go completely flat the battery could be dead forever.

Not necessarily. The battery drains even when not in use so might as well keep the phone on with it.

If you power off when you sleep at night and when you're in meetings, an old flip phone could last several years without battery change, but a modern smart phone, fewer years.
