How do doctors go about diagnosing you with a mental illness like depression? Do they monitor you or ask you questions or something ?

Psychiatrists ask you a lot of questions about your symptoms, mood, and family history of mental illness and then make their diagnosis based on the things you tell them.

they might do both of that

Doctors are not trained to diagnose mental illness. They can treat short term self claims with light meds, but that is as far as it goes with a M.D.
To be diagnosed, you need a psychologist or psychiatrist. These two types of specially trained experts talk to you, ask you questions and evaluate your answers, body language, tone of voice, eye movements and much more.
It's all one on one time.
