how can my doctor know i need to drink more water and not soda from an ekg?

He can only assume, He's probably telling you to drink more water, because its good for you.

From your heart rhythm. Soda does not hydrate as well as just plain water, especially if it contains caffeine which is diuretic. There's a tipping point in consumption of caffeinated beverages where they become more diuretic than hydrating. Caffeine also affects heart rhythm. Your doctor probably sees this a lot.

Because dehydration shows up on an EKG. And soda does not rehydrate like water. I had an EKG 3 months ago and my doctor said the same thing.

It was probably determined through a blood test. Your sodium level was probably very high.
