Atrial fibrillation linked to an increased risk of dementia. Why are some patients not receiving anti-coagulation?

They should be receiving anti coagulants because of the risk of stroke, not dementia. I can't imagine why any a-fib patient wouldn't be prescribed blood thinners unless they have some kind of advanced directive that prevents them from being prescribed any.
