It was a compliment. No need to say anything.

You did the right thing, i would have been tempted to say " if you took the stairs now and then you might not be so fat!"

There was actually nothing to say to her. she was looking for a complement

Clearly an explaination that the woman didn't have to feel bad about herself.

You should warn her that elevators have weight limits , & then suggest she take the stairs. The added bonus is each step she takes, both up & down burns caleries. That would be very helpful of you if you encounter the fatty again.

Well, it's a bit late now to say anything. In hindsight, you could have said you used to be fat until you started to diet and exercise.

I think she was trying to be funny. I would just chuckle in a friendly manner and go about my way. I might also say "oh no not at all". It's vague enough that you can't tell if I'm saying it about myself being thin or about her being fat.
