Your percentile score on GMAT is only 82%ile. That's not great. How were your undergraduate grades? What promotions have you received over the past 5 yrs? Does your employer recommend you as prospective managerial-executive caliber talent? Does your employer encourage you to pursue MBA, offering any tuition reimbursement plan, or proposing future promotions if you do well in MBA?
Given the glut of MBAs on most markets, it does not make much sense to go for MBA unless you are a talented, highly qualified candidate. If you are determined to go, you will have to research admissions requirements for schools you are interested in, check statistics on admitted students. If you do not exceed average GPA, GMAT scores/percentiles, etc, for a particular school, there's no point applying.
