The saying is not meant to be literal. THE POINT is to give a high priority to customer satisfaction.

No, sometimes they re wrong but since they are customer we should treat them with respect and give them sympathy.

Nope. Years ago, I worked as a clerk in a store. This woman bought a Christmas bulb which she gave to her bratty kid, who promptly threw it on the floor (not accidentally dropped, it, but THREW it) and the lady said "I want to return this; it's broken"!

Nope. Some times you have to draw the line. But you should for the most part try to help the customer unless there is nothing that you can realistically do to make them happy.
