it starts by the fact that the pharmaceutical lobby is the largest political lobby in US congress of which US senators are legally taking "legal bribes" (aka campaign contribution) regularly.. bigger than any other lobby - bigger than oil and bigger than the coal industry.. so the Senate would shoot down any legislation that may jeopardize their multi-billion earnings..
most other countries (including what you would call 3rd world countries) put a cap on how much a drug is sold and how much those big pharma could earn from a much needed drug.. and if one company sells its drugs beyond what deems to be reasonable, that country would encourage the competition to reverse-engineer the drug and produce the same drug.. trade-mark, patents and law-suits would not matter much within the borders of those countries..

Because the Medical and Pharmaceutical Industries make huge contributions to politicians who will prevent legislation requiring Life Saving Drugs be sold at affordable prices.

Because demand is fairly inelastic, meaning that the manufactures can raise prices to profiteering levels without losing many sales.
America does not have a health care system. America has a very profitable medical products & services industry.
