Goals now and Goal a year from now.
Deal with your psychological issues, since ppl who gain a tremendous amount of weight such as yourself, could have been traumatized by something, as a child or by someone, and they are just comforting themselves.
Personally, I am struggling with weight, but not by that amount, personally, ate to comfort myself...
Deal with issues, by getting professional help, and get help while eating healthy and not blaming oneself for issues that we faced as children.
In the meanwhile, since your sister is your help, work with her, and hear what she says....things will always look up, when we set goals, and even when we fall, we get right back up, and try again.
Only your sis can answer this, because you need family around you, for now.
