    The date of the Notice, of course.

    There may be an additional date which is the Period Date. This is the Period the notice is regarding.

    The period is a week, month, Quarter (3 month period) or year. All tax payments or returns cover a specific period. For example, the 1040 covers a calendar year (like 2013) while a 941 (Payroll Return) covers a Quarter (3 month period).
