What does "twice up the barrel, once down the side" mean?

I heard someone use this phrase in a business meeting yesterday and everyone but me seemed to know what it meant.

    The Urban Dictionary says it means that something is a sure thing. Doesn't make sense to me. But here is a link to a blog that is supposedly written by the person who "invented" the phrase:
