Being Batman is a physically demanding job, Bruce Wayne had been shot, beaten, set on fire, gassed by weaponized hallucinogens, fallen from extreme heights, bitten by dogs, survived explosions, crashes, etc.. While the Batsuit would offer him some protection, the physical trauma he had sustained would certainly take its toll on his body. As we see at the beginning of The Dark Knight, Bruce had several scars over his body and he had only been Batman for less than a year. In this film, 8 years have passed and Bruce has been locked away in seclusion. If he didn't stay in top shape, the previous trauma to his body would likely start taking their toll. At the end of The Dark Knight, Bruce can be seen limping after falling while running from the police, so the cane is probably a result of the fall.

I think it's because of the fall at the end of The Dark Knight where he tries to save Commissioner Gordon's son. Or it could even be because of his age (the movie is set 8 years after The Dark Knight).

You have to watch Batman Begins and The Dark Knight,,, He got shot by Harvey Dent in the previous movie and fell off a building, then Jim Gordon told everyone that Harvey Dent was murdered by the batman after the batman told him to do it,,,, he never went to a doctor,,,
