Who bears the burden when paying for orphan drugs?

Orphan drugs generally cost in the hundreds of thousands. One of the most expensive, Soliris has a price tag of over $400,000.

In America, who pays the price? For those with private insurance, what percentage do the insurance companies pay, and how much is left in copays?

For the uninsured, are they left to go without treatment?

Will medicare or medicaid help?
