Warning system\Early Warning Systems (EWS) were notoriously error-prone. For example, on 78 occasions in 1979 alone, a "missile display conference" was called to evaluate detections that were "potentially threatening to the North American continent". Some of these were trivial errors and were spotted quickly, but several went to more serious levels. On September 26, 1983, Stanislav Petrov received convincing indications of a US first strike launch against the USSR, but positively identified the warning as a false alarm. Though it is unclear what role Petrov's actions played in preventing a nuclear war during this incident, he has been honored by the United Nations for his actions.
The world came unusually close to nuclear war - although perhaps not as close as during the Cuban Missile Crisis - when the Soviet Union thought that the NATO military exercise Able Archer 83 was a ruse or "cover up" to begin a nuclear first strike. The Soviets responded by raising readiness and preparing their nuclear arsenal for immediate use. Soviet fears of an attack went away once the exercise concluded without incident.

I'm sure the Russians thought it was close to a nuclear war far more often than us.
Curtis Lemay would send our B-52s far into Russia as if they were on a nuclear bombing run. The bombers would be recalled before Russian fighters could reach them but the Russians didn't know it was just a Curtis Lemay parlor game. They thought it was the real thing.
Curtis as commanding officer of the Airforce constantly asked the president for the go ahead to pre-emptive sneak attack on Russia. The guy was a nutcase and the VP running mate of George Wallace. Imagine what the world would be like now had Wallace been elected then killed. Lemay would have attacked Russia on his second day in office.

in 1906 a meter hit in siberia with a force that was greater than an atomic bomb
if that had happened in 1976 the USSR would have retaliated immediately, no questions asked
when the radar was set up to warn of USSR missiles it Saw and attack coming" fortunately thhe generals realized it was so unlikely given the situation at the time they held fire and it was found the radar was seeing the moon from signals sent many seconds ago
wee may never know how close we came to the last war everSource(s): tested Nukes in the 60s
