    If you're looking for your 1099-G from 2011 the information is available online. Don't clog up the phone system just to ask for your 1099.

    When you file a new claim by phone, we first ask you a series of questions (PDF, 35KB), which you will answer using the keypad on your telephone.

    After you answer the questions, we will transfer you to a claims specialist to complete your claim. The actual number of questions you are asked varies depending on the type of claim you are filing and how you answer the questions.

    If you are directed to mail or fax something to the claims center, make sure you do so.

    Every January we begin mailing Form 1099-G (Statement for Recipients of Unemployment Compensation Payments) to people paid unemployment insurance (UI) during the prior calendar year.

    Starting the middle of January through April 30 each year you can also get your prior year tax information:

    If you were issued more than one 1099-G, you will hear a total amount that includes all 1099-Gs.

    Address changes that occurred after December 31 are not reflected on the 1099-Gs. The post office will forward 1099-Gs, but only if you provide them with a forwarding address.
