It isn't the most offensive per se but a lot of people are offended by the "plug and socket" costumes for men and women as well as the sperm and egg costumes. At a college party once I saw a guy dressed up as a sperm in a condom, not offensive necessarily but funny and "crude." First thing that comes to mind would be a costume of Hitler or Bin Laden. I suppose anything related to real-life mass murderers would be considered tactless.

I think costumes that are just plain lame that have little or no thought put into them.Like a t-shirt that says "this is my costume"
