    http://tnid.us/ will tell you the current provider, as well as the original provider (in the case that the number has been ported) as well as CNAM info for a number (if it is available)

    keep in mind, it will provide information for the actual carrier, not necessarily the provider that the customer is getting the service from - as an example if you look up a google voice number it will show the carrier as broadband.com because that is the carrier that google purchases there services from and then provides to their customers... a phone line that is provided by a company that resells Verizon's land line service (a CLEC) might show as Verizon being the carrier, if the company is using Verizon's switch...
