    You would need to identify the country and region you want this information for. In the US alone, this will vary by state, and many of these companies probably offer their employees multiple choices in each region.

    Nobody for most, if not all of them. Large companies don't buy insurance. They pay an insurance company to use their network and administer claims. They pay for the costs out of their corporate pockets.

    i suspect that many of them are self-insured and that the paperwork is contracted out to some insurance operation, which fronts for them. The law firm may not be large enough to do that.
