Where should you change your baby's diaper if a restaurant doesn't have a changing table?

I've been in this situation several times, and I'm wondering how other moms or dads have handled it. What do you do when you are at a restaurant that does not have a changing table in the bathroom (and does not have any counter space or anything in the bathroom that could even be used as a changing table),... show more I've been in this situation several times, and I'm wondering how other moms or dads have handled it. What do you do when you are at a restaurant that does not have a changing table in the bathroom (and does not have any counter space or anything in the bathroom that could even be used as a changing table), but your baby desperately needs a diaper change? Assume you do not have a car with you or a stroller that reclines enough to accommodate a diaper change.

Do you:
a. Change your baby on the floor of the bathroom?
b. Change him/her outside on a bench or other surface?
c. Change him/her on a table in the restaurant?
d. Change him/her on a chair in the restaurant?
e. Have any other ideas?

Now, does your answer change depending on the conditions?
1. What if the bathroom floor is filthy?
2. What if it's very cold outside?
3. What if it is rainy or snowy outside?
4. What if the diaper is just very wet?
5. What if the diaper is very poopy and stinky?

Bonus Question: If you have ever been in this situation, what reactions have you gotten from other restaurant patrons?
Update: EDIT: Remember, the car is not an option in this scenario (either because it's parked numerous blocks away, as is often the case when dining out in an urban area, or because you walked or took public transportation to your destination).
