Easiest way to check if your trans is slipping is to press hard on the brake and hit the gas. The RPMs will go up but if the rpm gauge just shoots up and can go to redline its slipping. If it does not do this you have other problem(s). Everything from a broken motor mount to a differential mount can cause this jerking feeling. Really there are maybe 100 reasons you are experiencing this jerking and not all have anything to do with the drive train at all.

You can try to change the transmission fluid and filter now, but if it doesn't help, it is probably too late to save your transmission. Time for a tranny job. $$$

Alot makers claim their seal transmissions never need servicing or changing fluid that is not true. The transmission specialist recommend you service your auto transmission or manual every 80,000k or 50,000miles. Get it serviced and have them adjust the bands and change the fliud at a transmission euro workshop.
