local access number = a number in a local area code dialed to use the calling card... local access numbers usually get you better rates than using a toll free access number, but if you do not have unlimited local calling, or are calling from a payphone you may have to pay for making this local call..

toll free access number = a number in a toll free area code dialed to use the calling card, more convenient than a local access number when needing to call from a payphone, but normally deducts a connection fee, and/or higher rates for the call than a card that uses a local access number

The "local" number called is within your geographical area and serviced by the phone company you're doing business with. The "access" number may actually involve a different phone companies lines in order to complete your call under the plan of the company you normally use.
