Those four words roll off the tongue as a lighthearted way of asking "What's up? What's next?" And that tripping lightly stuff makes sense, since "How now, brown cow" has its origin in elocution, where the phrase was used to demonstrate properly rounded vowels. Each "ow" sound in the phrase represents an individual diphthong.

It depends on the circumstances but it generally means he is glad you didn't tell on yourself.

It either means he's glad you didn't come out of the closet or that you didn't commit suicide.

The context in which this was said is important to interpret the underlying meaning.

It means that the person speaking is glad that you didn't betray whatever secret that he/she believes you're hiding.
