Professor Wikipedia says it was Forrest J Ackerman in 1954. It was originally used in a pejorative sense, to describe pulpy magazine stories and movies that had some of the trappings of science fiction (aliens, robots, spaceships, rayguns) but didn't really understand what the genre was supposed to be about. It was pronounced "skiffy" rather than "sigh fye"

I don't know, but it stands for Science Fiction so I'm just guessing a lot of people happened to use it and everyone just used it after a few credible authors used it to describe their work.

I'd be interested to find out though- if there was some sort of formal announcement regarding the subject.

Probably some moronic marketing "Wunderkind" who couldn't, or was to lazy to, spell out science fiction.
